Second Stage Labor Management

    Risk and malpractice during the second stage of labor has increased during the last decade. Improvements in interventions to maximize maternal and fetal oxygenation during this time are key for postitive patient outcomes. This course incorporates BESST practice guidelines and recommendations to promote fetal descent, oxygenation, and vaginal success.

    Traditional therapies render the second stage of labor the most anoxic phase of labor and birth for both mother and fetus. Populations at risk for desaturation are highlighted. Second stage contraction intensity and frequency may negatively influence morbidity. Strategies outlined to maximize maternal-fetal saturation include: open-glottis pushing, vertical positioning, alternate pushing, laboring down, and patience. A a collaborative approach is discussed and outlined for enhanced patient safety & vaginal delivery success. This course includes evidence-based resources to define BESST Guidelines for optimum maternal-fetal oxygenation during second stage. ACOG, AWHONN, and others support laboring down/passive pushing when clinically indicated for all patients during this period. This course may be taken as a single topic for 2.4 CNE or as one of five lessons included in the Perinatal Qualilty, Risk, & Safety Initiative bundle course.
